Carbamazepi ne Neurol og ic ma ni festat i on : 2 c a se r eports Substitution of carbamazepine for valproic acid produced mood swings , episodes of aggressive behaviour, acoustic hallucinations, paranoid ideation and thought disorder in a 24- year-old man with epilepsy and mild mental handicap. The patient 's mental awareness and consciousness remained normal. Non-toxic concentrations of serum carbamazepine were observed. Carbamazepine dosage was tapered after 6 months' treatment and valproic acid was reintroduced . Pronounced sustained improvement of mental state and behaviour was observed 1 week after carbamazepine withdrawal. A severe phobia of water developed in a 5-year-old epileptic girl administered carbamazepine. The condition resolved following discontinuation of the drug. Because people with brain damage or mental impairment are sensitive to marginally elevated anticonvulsant concentrations , ' .. . psychiatric symptoms induced by carbamazeplne at non-toxic concentrations may occur as idiosyncratic reactions' in these patients. Carbamazepine Neurologic manifestation: 2 c ase reports
