First report of retrograde ejaculation: case reportA 35-year-old man experienced retrograde ejaculation while
receiving oxcarbazepine to treat post-traumatic epilepsy.The man starting having weekly seizures about 1 year after a
brain injury and was initiated on oxcarbazepine, which wasgradually titrated up to 1800 mg/day [route not stated]. Whileon titration, he reported difficulty in achieving ejaculation, andpresented with complete anejaculation a few days afterreaching the maximal dosage. His sexual history wasunremarkable, and his sexual drive and erections wereunaltered. He noted that his postorgasm urine was foamy, andurinalysis revealed the presence of semen. Iatrogenicretrograde ejaculation was suspected.
The man’s oxcarbazepine therapy was graduallydiscontinued, and topiramate was initiated without seizures orsexual side effects. One week after oxcarbazepine waswithdrawn, he presented with anterograde ejaculation, and nosemen was found in his urine at 30 days’ follow-up.

» Editorial comment: A search of AdisBase, Medline andEmbase did not reveal any previous case reports of retrogradeejaculation associated with oxcarbazepine. The WHO ADRdatabase did not contain any reports of retrograde ejaculationassociated with oxcarbazepine.
