Reactions 1434 - 12 Jan 2013 Oxcarbazepine First report of retrograde ejaculation: case report A 35-year-old man experienced retrograde ejaculation while receiving oxcarbazepine to treat post-traumatic epilepsy. The man starting having weekly seizures about 1 year after a brain injury and was initiated on oxcarbazepine, which was gradually titrated up to 1800 mg/day [route not stated]. While on titration, he reported difficulty in achieving ejaculation, and presented with complete anejaculation a few days after reaching the maximal dosage. His sexual history was unremarkable, and his sexual drive and erections were unaltered. He noted that his postorgasm urine was foamy, and urinalysis revealed the presence of semen. Iatrogenic retrograde ejaculation was suspected. The man’s oxcarbazepine therapy was gradually discontinued, and topiramate was initiated without seizures or sexual side effects. One week after oxcarbazepine was withdrawn, he presented with anterograde ejaculation, and no semen was found in his urine at 30 days’ follow-up.
