Reactions 1479, p25-26 - 23 Nov 2013 S Oxcarbazepine Complete atrioventricular block in an elderly patient: case report A 67-year-old woman developed complete atrioventricular (AV) block while receiving oxcarbazepine for partial seizures. The woman presented to an emergency service with severe headaches and fatigue. Her symptoms had been intermittent since she started receiving oxcarbazepine 300mg twice daily 2 years earlier [route and time to reaction onset not stated]. On admission, cardiac examination found bradycardia; following an ECG, she was transferred to a cardiology clinic with a diagnosis of complete atrioventricular block. The woman received a temporary pacemaker. ECG found grade 1 diastolic dysfunction, and oxcarbazepine was withdrawn; she started receiving levetiracetam. Three days later, she developed 2:1 AV block followed by normal sinus rhythm. No arrhythmia was detected on 24-hour Holter monitoring. The pacemaker was removed, and she was discharged. No bradycardia or conduction disorders were found during follow-up. Author comment: "[A]nother reason of complete AV block might be the effect of [oxcarbazepine] on potassium channels." The woman presented to an emergency service with severeheadaches and fatigue. Her symptoms had been intermittentsince she started receiving oxcarbazepine 300mg twice daily2 years earlier [route and time to reaction onset not stated].
