Reactions 1408 - 30 Jun 2012 Lorazepam First report of paresis of accommodation: case report A 23-year-old man developed paresis of accommodation in both eyes during treatment with lorazepam [Ativan] for depression. The man received lorazepam 1.5 mg/day [route not stated] alongside fluoxetine therapy. He developed near- sight disturbance in both eyes 5 days after starting lorazepam but did not present until 2 weeks after treatment initiation. Visual examination revealed visual acuity of 1.0 in both eyes but disturbed near point of accommodation (NPA); 16cm OD and 19cm OS. Sluggish pupillary reflex was also evident in both eyes. The man’s near-sight disturbance was relieved by near glasses correction. Lorazepam was discontinued 7 days after presentation and replaced with diazepam. NPA 15 days after lorazepam cessation had improved to 14cm OD and 12cm OS. Recovery of NPA to 6cm in both eyes was evident 2 months after drug discontinuation. Author comment: "In the present case, near-sight disturbance with prolonged NPA was noted and the symptom was relieved after cessation of lorazepam.
