Hyperammonaemia and asterixis: case reportA 30-year-old woman with bipolar disorder developed
hyperammonaemia and asterixis following treatment withcarbamazepine.
The woman developed impaired sensorium and jerks inboth upper limbs with asterixis within 2 weeks of achievinga therapeutic dose of carbamazepine of 600mg [dosingfrequency, route and therapeutic indication not stated].Laboratory tests were within normal limits except for anelevated serum ammonia level of 119 Umol/L. Noalternative contributory comorbidity or concomitantmedication was identified.
Carbamazepine was tapered then discontinued. Thewoman’s symptoms of jerks in her upper limbs withasterixis reduced and she regained her sensorium. Herserum ammonia level subsequently decreased to47 Umol/L (normal range 11–35 Umol/L).
