Reactions 1412 - 28 Jul 2012 Carbamazepine Hyperammonaemia and asterixis: case report A 30-year-old woman with bipolar disorder developed hyperammonaemia and asterixis following treatment with carbamazepine. The woman developed impaired sensorium and jerks in both upper limbs with asterixis within 2 weeks of achieving a therapeutic dose of carbamazepine of 600mg [dosing frequency, route and therapeutic indication not stated]. Laboratory tests were within normal limits except for an elevated serum ammonia level of 119 Umol/L. No alternative contributory comorbidity or concomitant medication was identified. Carbamazepine was tapered then discontinued. The woman’s symptoms of jerks in her upper limbs with asterixis reduced and she regained her sensorium. Her serum ammonia level subsequently decreased to 47 Umol/L (normal range 11–35 Umol/L). Author comment: "We report a case of carbamazepine induced hyperammonemia with asterixis". Singh H, et al.
