Reactions 1343 - 19 Mar 2011 S Carbamazepine Toxic epidermal necrolysis: case report A 46-year-old woman, who had previously undergone brain surgery for meningioma, presented with toxic epidermal necrolysis (TEN) with >40% detachment of the body surface area. She had been receiving carbamazepine [route, dosage, therapeutic indication and time to reaction onset not stated], hydantoins, cotrimoxazole [trimethoprim/sulfamethoxazole] and pantoprazole over the previous 3 weeks. Her symptoms completely resolved. Three months later, a patch test with carbamazepine was positive. Author comment: "In this patient the patch test reproduced clinically and on histology the type of drug eruption induced by systemic drug exposure, with extensive epidermal necrosis and infiltration of CD8+ and CD56+ T cells rich in granzyme B, as usually reported in TEN." Goncalo M, et al. Positive patch test in toxic epidermal necrolysis, with clinical and histological aspect typical of ten. Contact Dermatitis 63 (Suppl. 1): 22 abstr. FS5.06, Sep 2010.
