Reactions 728 - 21 Nov 1998 Carbamazepine Abnormal pitch perception: case report A 16-year-old girl reported lowering in her perceived pitch of sounds 3 days after she started carbamazepine 200 mg/day for epilepsy. The girl noted lowered pitch of the ring of a telephone, music and other everyday sounds. Two weeks later, after her carbamazepine dosage was increased to 300 mg/day, she developed tinnitus. She denied decreased hearing ablility, although no formal audiological tests were performed. Four weeks later, she still had auditory disturbances. At this time, her serum concentration of carbamazepine was 4.3 µg/ml. Carbamazepine was stopped and replaced with zonisamide. The girl’s auditory symptoms resolved 2 days later. Author comment: ‘Because auditory disturbance may occur at a therapeutic serum level of CBZ [carbamazepine], heightened awareness of reversible hearing impairment upon starting CBZ is important, especially when treating younger patients who may not complain of the symptom.’ Kashihara K, et al.
