Reactions 614 - 17 Aug 1996 S Benazepril First report of angioneurotic oedema in an elderly patient: case report A 65-year-old woman developed angioneurotic oedema during treatment with benazepril for hypertension. After 14 months’ therapy with benazepril 10 mg/day, the woman was hospitalised with tongue swelling. Benazepril and her other medications (clonidine, hydrochlorothiazide, atenolol and conjugated estrogens) were stopped. Despite treatment with oral diphenhydramine, IV methylprednisolone and ranitidine, the tongue swelling continued. She began to have difficulty swallowing and speaking. SC epinephrine [adrenaline] was given and the oedema improved. The woman was then treated with IV dexamethasone and IV ranitidine over a 2-day period and her angioneurotic oedema continued to improve. She restarted treatment with all of her usual medications, except for benazepril. The woman was discharged 48 hours after hospital admission, and remained well at follow-up 22 months later. Author comment: Angioneurotic oedema has been reported in patients receiving captopril, enalapril, lisinopril, quinapril and ramipril. However, there have been no previous published reports of angioneurotic oedema associated with benazepril. The package insert for benazepril states that angineurotic oedema occurred in about 0.5% of patients who received the agent in US clinical trials. O’Mara NB, et al.
