Reactions 1353 - 28 May 2011 Antineoplastics Neutropenia and herpes zoster: 2 case reports Two women with breast cancer developed neutropenia and herpes zoster during treatment with FAC (cyclophosphamide, doxorubicin [adriamycin] and fluorouracil) ± docetaxel [routes, dosages and durations of therapy to reactions onset not stated]. A 68-year-old postmenopausal woman began receiving chemotherapy according to the FAC regimen. After 4 cycles, she presented with a fever. She had a WBC count of 1.2 × 10 3 /mL, with a neutrophil count of 0.3 × 10 3 /mL and a lymphocyte count of 0.6 × 10 3 /mL. She was treated with amikacin, ceftazidime and filgrastim. Two days after starting filgrastim, she developed painful erythematous lesions on the right side of her body. A diagnosis of herpes zoster was made, and aciclovir was added to her treatment regimen. Her neutrophil and lymphocyte counts normalised after 4 days of filgrastim administration, with resolution of the lesions within 2 weeks. A 43-year-old premenopausal woman began receiving docetaxel after 4 cycles of FAC chemotherapy. After the first cycle of docetaxel, she developed neutropenia. Prophylactic filgrastim was started after the second cycle of docetaxel. She developed painful erythematous and vesicular lesions 3 days after starting filgrastim, and herpes zoster was diagnosed. Her WBC, neutrophil and lymphocyte counts were normal. Lidocaine was applied to the lesions for 1 week, and they resolved within 2 weeks. Sayin M, et al.
