Neutropenia and herpes zoster: 2 case reportsTwo women with breast cancer developed neutropenia
and herpes zoster during treatment with FAC(cyclophosphamide, doxorubicin [adriamycin] andfluorouracil) ± docetaxel [routes, dosages and durations oftherapy to reactions onset not stated].
A 68-year-old postmenopausal woman began receivingchemotherapy according to the FAC regimen. After4 cycles, she presented with a fever. She had a WBC countof 1.2 × 103/mL, with a neutrophil count of 0.3 × 103/mLand a lymphocyte count of 0.6 × 103/mL. She was treatedwith amikacin, ceftazidime and filgrastim. Two days afterstarting filgrastim, she developed painful erythematouslesions on the right side of her body. A diagnosis of herpeszoster was made, and aciclovir was added to her treatmentregimen. Her neutrophil and lymphocyte countsnormalised after 4 days of filgrastim administration, withresolution of the lesions within 2 weeks.
A 43-year-old premenopausal woman began receivingdocetaxel after 4 cycles of FAC chemotherapy. After thefirst cycle of docetaxel, she developed neutropenia.Prophylactic filgrastim was started after the second cycle ofdocetaxel. She developed painful erythematous andvesicular lesions 3 days after starting filgrastim, and herpeszoster was diagnosed. Her WBC, neutrophil andlymphocyte counts were normal. Lidocaine was applied tothe lesions for 1 week, and they resolved within 2 weeks.Sayin M, et al.
