Thrombotic microangiopathy: case reportA 31-year-old woman with Hodgkin’s disease developed
thrombotic microangiopathy during treatment withantineoplastics [specific drugs, dosages and duration oftreatment not stated].
The woman, who had previously received eight cycles ofantineoplastic therapy [specific drugs, dosages and duration oftreatment not stated], started receiving high-doseantineoplastic therapy and autologous stem celltransplantation. Six months after autologous bone marrowtransplantation, she developed blurred vision, dyspnoea and atemperature between 38 and 38.5°C [time to reaction onsetnot stated]. Ophthalmogical examination showed severe lossof vision with a visual acuity of 20/400 in both eyes. Dilatedfundus examination revealed bilateral cotton-wool spotsaround her peripapillary area. She had bilateral scatteredretinal haemorrhages and markedly reduced retinal arterioles.She also had a cherry-red spot in her right eye. She was initiallydiagnosed with a Purtscher-like retinopathy. Laboratoryinvestigations revealed severe anaemia (haemoglobin 6 g/dL),thrombocytopenia (platelet count 50 000 × 109/L) and anelevated C-reactive protein levels (197). A chest x-ray revealedbilateral consolidation of her lower lobes and renal biopsyfindings were consistent with thrombotic microangiopathy.She was provisionally diagnosed with systemic vasculitis.
The woman received IV methylprednisolone and hernephropathy and retinopathy appeared to stop progressing. Awork-up for vasculitis was negative and it was concluded thatclinical and biological findings supported a diagnosis ofthrombotic microangiopathy. [Patient outcome not clearlystated.]Parc C.
