Reactions 1249 - 25 Apr 2009 S Antineoplastics Oesophageal stricture (first report with mercaptopurine and tretinoin) in a patient with Down syndrome: case report A 16-year-old boy with Down syndrome developed oesophageal stricture while receiving maintenance antineoplastic therapy [dosages and durations of treatments not stated] for acute promyelocytic leukaemia; he had also experienced severe mucositis during induction therapy. The boy initially received induction therapy with cytarabine, daunorubicin and tretinoin [all-trans retinoic acid], but this phase of treatment was complicated by severe mucositis, febrile neutropenia, respiratory distress associated with tretinoin syndrome, and tumour lysis syndrome with acute renal failure. Once he had recovered, he received consolidation therapy with daunorubicin and reduced-dose tretinoin, followed by maintenance therapy with mercaptopurine, oral methotrexate and tretinoin. While receiving maintenance therapy, he developed nausea and vomiting. He was treated with maximal antiemetic therapy with no improvement. The boy’s chemotherapy doses were reduced, but his vomiting continued and maintenance therapy was discontinued 9 weeks early. Routine echocardiography a few weeks later revealed a posterior mediastinal mass. A chest CT scan revealed moderate upper thoracic oesophageal dilatation, and lower oesophageal narrowing and thickening. A barium swallow confirmed significant narrowing of his distal oesophagus, and upper GI endoscopy revealed a severe inflammatory stricture resulting in a luminal diameter of approximately 5mm. An oesophagectomy with gastric conduit reconstruction was performed, and a pathological examination of the stricture revealed fibrosis secondary to chronic inflammation. He was able to tolerate oral feeding following surgery and he regained bodyweight; however, he experienced leukaemia relapse 26 weeks after the end of his treatment, and he died.
