Antineoplastics

Myocardial infarction: case reportAn acute myocardial infarction occurred in a 32-year-old

man without cardiac risk factors following initiation ofstandard antineoplastic therapy for Hodgkin’s disease. Thevinca alkaloids in the antineoplastic regimen were implicated.

The patient experienced nausea, diaphoresis and diffusechest tightness with radiation within 2 hours of receivingchlormethine and vincristine on day 1. Seven days later, he hadan acute myocardial infarction 5 hours after receivingdoxorubicin, bleomycin and vinblastine. His treatmentregimen also included prednisone and procarbazine for 10-14days. The patient received calcium antagonist and ACEinhibitor therapy and was asymptomatic at follow-up 12months later.

’Awareness of the potential for vinca-induced cardiotoxicity. . and its timely treatment are at present the best defense’against this potential complicationHouse KW, et al.
