Reactions 394 - 28 Mar 1992 Antineoplastics Myocardial infarction: case report An acute myocardial infarction occurred in a 32-year-old man without cardiac risk factors following initiation of standard antineoplastic therapy for Hodgkin’s disease. The vinca alkaloids in the antineoplastic regimen were implicated. The patient experienced nausea, diaphoresis and diffuse chest tightness with radiation within 2 hours of receiving chlormethine and vincristine on day 1. Seven days later, he had an acute myocardial infarction 5 hours after receiving doxorubicin, bleomycin and vinblastine. His treatment regimen also included prednisone and procarbazine for 10-14 days. The patient received calcium antagonist and ACE inhibitor therapy and was asymptomatic at follow-up 12 months later. ’Awareness of the potential for vinca-induced cardiotoxicity . . and its timely treatment are at present the best defense’ against this potential complication House KW, et al.
