Acute myeloid leukaemia in a child?: case reportA 3-year-old boy with atypical teratoid/rhabdoid tumour

of the brain, who was conceived using in vitro fertilisation,developed acute myeloid leukaemia after treatment withetoposide, ifosfamide, vincristine, dactinomycin andcyclophosphamide.

The boy underwent subtotal removal of a mass aroundthe posterior fossa. Histologically, the mass was consistentwith atypical teratoid/rhabdoid tumour. He receivedinduction therapy according to the IntergroupRhabdomyosarcoma Study protocol includingIV ifosfamide (cumulative dose, 54 g/m2), IV etoposide(cumulative dose, 3000 mg/m2), vincristine [dosage notclearly stated], dactinomycin [dosage not clearly stated],IV cyclophosphamide (cumulative dose 12 g/m2), andradiotherapy. Seventeen months after the initial diagnosis,he had a haemoglobin level of 9.3 g/dL, a leukocyte count of80 300/mm3 (with 17% blasts, 39% neutrophils, 21%monocytes and 8% lymphocytes) and a platelet count of51 000/mm3 [duration of therapy to reaction onset notclearly stated]. Blasts and myelodysplastic features, withpositivity for myeloperoxidase staining, were observed inbone marrow aspirate. Positivity for CD13 and CD33 wasalso found and he had a bone marrow karyotype of 46, XY.

The boy had developed secondary or therapy-related M2acute myeloid leukaemia. He was started on anti-leukaemictherapy. [Patient outcome not stated].
