First report of migraine exacerbation during cross-over from carbamazepine: 3 case reports
Three women with epilepsy developed migraines whileswitching from carbamazepine to oxcarbazepine.
A 33-year-old woman, with a history of sumatriptan-responsive migraines, was switched from carbamazepine1200 mg/day to oxcarbazepine 2100 mg/day. During cross-over, she experienced a refractory migraine, with a poundingheadache, nausea and vomiting, lasting 4 days. Oxcarbazepinewas discontinued and carbamazepine reintroduced, withresolution of her migraine.
A 38-year-old woman, with a history of mild migraines, wasswitched from carbamazepine 1600 mg/day to oxcarbazepine2400 mg/day. During cross-over, she developed a poundingheadache and vomiting. Her migraine was refractory totreatment, but resolved after discontinuation of oxcarbazepineand reintroduction of carbamazepine.
A 30-year-old woman, with a history of migraines treatedwith dipyrone, caffeine and isometheptene, was switchedfrom carbamazepine 1200 mg/day to oxcarbazepine1800 mg/day. After 7 days, she developed a migraine thatpersisted for 4 days and resolved after oxcarbazepinediscontinuation and carbamazepine reintroduction.Amaral V, et al. » Editorial comment: A search of AdisBase and Medline didnot reveal any previous case reports of migraine associated withoxcarbazepine. The WHO Adverse Drug Reactions database tenreports of migraine associated with oxcarbazepine.
