Reactions 1086 - 28 Jan 2006 Oxcarbazepine First report of migraine exacerbation during cross- over from carbamazepine: 3 case reports Three women with epilepsy developed migraines while switching from carbamazepine to oxcarbazepine. A 33-year-old woman, with a history of sumatriptan- responsive migraines, was switched from carbamazepine 1200 mg/day to oxcarbazepine 2100 mg/day. During cross- over, she experienced a refractory migraine, with a pounding headache, nausea and vomiting, lasting 4 days. Oxcarbazepine was discontinued and carbamazepine reintroduced, with resolution of her migraine. A 38-year-old woman, with a history of mild migraines, was switched from carbamazepine 1600 mg/day to oxcarbazepine 2400 mg/day. During cross-over, she developed a pounding headache and vomiting. Her migraine was refractory to treatment, but resolved after discontinuation of oxcarbazepine and reintroduction of carbamazepine. A 30-year-old woman, with a history of migraines treated with dipyrone, caffeine and isometheptene, was switched from carbamazepine 1200 mg/day to oxcarbazepine 1800 mg/day. After 7 days, she developed a migraine that persisted for 4 days and resolved after oxcarbazepine discontinuation and carbamazepine reintroduction. Amaral V, et al.
