Reactions 1466, p33-34 - 24 Aug 2013 S Oxcarbazepine Exacerbation of myoclonic seizures: case report A 44-year-old man, who had juvenile myoclonic epilepsy (JME), developed an exacerbation of myoclonic seizures while receiving oxcarbazepine. The man had a history of myoclonic jerks from the age of 13 years, as well as episodes of generalised tonic-clonic seizures. His seizures were not completely controlled with clonazepam. His myoclonic jerks were associated with a subjective manifestation described as "something in the stomach", often followed by difficulty in speaking. Because his ictal semiology suggested partial epilepsy, he started oxcarbazepine 900 mg/day [route not stated]. Approximately 4 weeks later, he was admitted with a dramatic worsening of his clinical condition. He had widespread, asynchronous and asymmetric myoclonic jerks, with a subjective feeling of confusion. He also had a language disturbance causing slowing-down and fragmentation of his speech. A video-EEG revealed a myoclonic status epilepticus with positive and negative myoclonus. The man received lorazepam, and his myoclonus and EEG abnormalities resolved. Oxcarbazepine was withdrawn, and he started levetiracetam. Two years later, he was seizure-free. Author comment: "[W]e describe a case of myoclonic status epilepticus (MSE) recorded by video-EEG in a patient with JME treated with oxcarbazepine . . . the dramatic exacerbation of the myoclonic seizures witnessed in our case was clearly related to [oxcarbazepine]".
