Reactions 424 - 24 Oct 1992 Carbamazepine overdose First report of hyponatraemia in a child: case report Hyponatraemia occurred in a 2.5-year-old boy after he ingested a massive overdose of carbamazepine. The boy was given 3.3% dextrose and 0.3% normal saline for over 6 hours prior to admission. Seven hours after ingestion of the overdose, his serum carbamazepine concentration was 129 micromol/L (therapeutic level 17-50 micromol/L). Following admission, the child received 5.0% dextrose and 0.9% saline for 7 hours, followed by 5% dextrose and 0.2% saline (duration of treatment unclear). Serum sodium and potassium levels measured 9 hours after carbamazepine ingestion were 131 mmol/L and 4.2 mmol/L, respectively. 19 hours after the carbamazepine overdose, the boy had a generalised tonic- clonic seizure lasting 30 seconds, and at this time his serum sodium level was 121 mmol/L and serum potassium level was 3 mmol/L. Author comment: ‘Acute intoxication with carbamazepine has not been reported previously to cause symptomatic hyponatremia ...
