Reactions 1365 - 20 Aug 2011 O S Carbamazepine overdose Unconsciousness and respiratory insufficiency: case report A 48-year-old man was admitted, unconscious and with respiratory insufficiency, after taking an overdose of carbamazepine. The man had been receiving carbamazepine 400mg twice daily for 10 years for a psychiatric illness; it was assumed to have been prescribed as a mood stabiliser. He had taken an unknown amount of carbamazepine, thought to be 150–200 tablets of carbamazepine 400mg. On arrival at an emergency department, he was unconscious and unresponsive, with respiratory insufficiency [time to reaction onset not stated]. The man was intubated and ventilated, and received gastric lavage and charcoal on admission. His serum carbamazepine level was 25.6 µg/mL. He remained unresponsive after 24 hours, so haemodialysis was initiated. After 2 sessions, he improved, and was weaned and extubated within the following 2 days. After a further dialysis session, his carbamazepine level was within the therapeutic range, and, 5 days after admission, he was discharged in a stable condition. Ram Prabahar M, et al.
