Reactions 1365 - 20 Aug 2011 O S Carbamazepine overdose Various toxicities: case report A 32-year-old woman developed various toxicities after a carbamazepine overdose; she was managed by haemodialysis. In a suicide attempt, the woman ingested carbamazepine 32g, and was brought to an emergency room 2 hours later. She was unresponsive and obtunded, with a Glasgow Coma Scale of 3, a heart rate of 94/min and a respiratory rate of 14/min. She also had hyporeflexia and flaccid muscle tone. Charcoal was administered. The woman later developed seizures, hypoxia and hypotension and was treated with vasopressors and fluids. She had a carbamazepine concentration of 32.9 µg/mL, 2.5 hours post-ingestion, and an acetaminophen concentration of 45 µg/mL. However, her carbamazepine concentrations continued to rise; due to this and her neurological and cardiovascular impairment, haemodialysis was commenced. 38 hours later she was awake with a carbamazepine concentration of 11.5 µg/mL; haemodialysis, ventilation and vasopressors were discontinued. She was subsequently discharged without sequel. Harder JL, et al. Carbamazepine and the active epoxide metabolite are effectively cleared by hemodialysis followed by continuous venovenous hemodialysis in an acute overdose.
