Reactions 1043 - 19 Mar 2005 S Carbamazepine First report of tonic-clonic seizures: case report A 20-year-old man with autism spectrum disorders experienced new-onset tonic-clonic seizures during treatment with carbamazepine. The man started treatment with carbamazepine 600 mg/day (serum concentration 8.5 µg/mL) for disruptive behaviours. He experienced a generalised tonic-clonic seizure approximately 2 months later, and three tonic-clonic convulsions were observed on 1 day 3 months later; an EEG showed bilateral and diffuse spike and waveforms. Carbamazepine was switched to valproic acid and the man’s seizures resolved. Valproic acid was tapered to discontinuation after 12 months and, during 10 months’ follow-up, no seizures or EEG abnormalities were observed. Monji A, et al.
