Carbamazepine First report of neutrophil Jeukocytosls Although the package insert for carhamazepine(Geigy) lists leuKocytosis as an adverse reaction to carbamazepine. no reports of neutrophilia were found in the literature. A 26-year-old Caucasian woman admitted for psychiatric e1(amination had been taking carbamazepine(600mg/dayl for seizures resulting from a car accident 4 years earlier. A routine hemogram showed a raised WBC coum of21.2 x lOll m l with 82 % neutrophils (normal 60-75 'K,). As it was noted thatleukocylosis was listed as an adverl\e effect of carbamazepine. her medication was changed to phenytoin (400mg I day) and phenobarbital ( I 2001&/ day I. Other drugs were continued unchanged chlorpromazine 125-300mg/day). an<l acetaminophen (paracetamol) with codeine (1·4 tablets/ day). Five days laler the WBC was normal. There were no signs of infection. Recause of dizziness and ataxia. phenytoin and phenobarbital were replaced by carbamazepine (600mgJ day )and WBC I I and I J days later were again significantly raised. \lurphy .
