Reactions 1374 - 22 Oct 2011 S Carbamazepine Fetal carbamazepine syndrome after in utero exposure: case report A female neonate was born with fetal carbamazepine syndrome after in utero exposure to carbamazepine, and later died. The girl was born by Caesarean section after 36.6 weeks’ gestation to a 33-year-old mother with epilepsy. The mother had taken oral carbamazepine 400 mg/day throughout the pregnancy. The girl showed abnormalities including generalised hypotrophy, a long head and narrow forehead, abnormal facial features, kyphosis, hypoplastic nails and a lumbosacral myelomeningocele. Neurological examination revealed hyporeactivity, lack of rooting and suckling reflexes, absent deep tendon reflexes in her legs, and absent anal tone. She developed respiratory distress soon after birth, and was intubated. An echocardiogram revealed pulmonary hypertension. Infection was suspected, and the girl was started on antibacterials. A blood count revealed neutropenia. On day 7 of life, she developed refractory respiratory alkalosis and died. Author comment: "We describe a newborn with several congenital malformations that died within the first week of life due to the severity of the abnormalities probably caused by in utero exposure to this drug."
