Reactions 1453 - 25 May 2013 S Carbamazepine DRESS: 2 case reports A 23-year-old man and his mother [age not stated] developed DRESS while receiving carbamazepine [routes, dosages and durations of treatment to reaction onset not stated]. The man received carbamazepine for epilepsy, and presented with DRESS. Three months later, his mother received carbamazepine for trigeminal neuralgia, and presented with a similar rash. Histological, haematological and biochemical features were consistent with DRESS. Carbamazepine was withdrawn, and both patients received steroid treatment. Their cutaneous features and systemic upset resolved. Carbamazepine-specific lymphocyte proliferation and interleukin-4 assays results confirmed the clinical suspicion of DRESS. Both patients were positive for the HLA-A*3101 allele, shown to be associated with carbamazepine-induced hypersensitivity. Author comment: "The association of anticonvulsant agents, including carbamazepine, with DRESS among other cutaneous reactions is well known . . . The man received carbamazepine for epilepsy, andpresented with DRESS. Three months later, his motherreceived carbamazepine for trigeminal neuralgia, andpresented with a similar rash. Histological, haematological andbiochemical features were consistent with DRESS.

Carbamazepine was withdrawn, and both patients receivedsteroid treatment. Their cutaneous features and systemicupset resolved.
