Skin disorders: 2 case reportsTwo patients treated for colonic adenocarcinoma with an
antineoplastic regimen comprising IV calcium folinate, IVfluorouracil and SC interferon alpha 2A, developed skin lesionswith histologic features characteristic of graft-versus-hostdisease.
The first patient, a 48-year-old woman, developederythematous papules on her forehead, palm and soles, thenhyperkeratosis of the palms and soles and hyperpigmentationof the forehead, 2 days after completing antineoplastictherapy.
The second patient, a 47-year-old man, noted progressivepigmentation of the palms, soles, face and upper back with aburning sensation in the palms during the second course ofantineoplastic therapy. With successive cycles ofantineoplastic therapy there was increased palmar and plantardysesthesia with peeling and fissuring of the skin andincreased hyperpigmentation. The patient was treated withpyridoxine for suspected palmar and plantarerythrodysesthesia with some improvement.
Histological examination of the skin biopsies in bothpatients showed apoptotic keratinocytes in the acrosyringiumand the adjacent epidermis with exocytosis of mononuclearcells and focal areas of hydropic degeneration of the basal celllayer.Beard JS, et al.
