Reactions 1234 - 10 Jan 2009 S Antineoplastics Pneumonitis: case report A 51-year-old man with non-Hodgkin’s lymphoma received five courses of R-CHOP: rituximab, cyclophosphamide, doxorubicin, vincristine and prednisone [dosages not stated]. F-fluorodeoxyglucose PET (F-FDG-PET) scanning showed diffuse lung uptake, and blood gas analysis revealed hypoxia with a decrease in PaO2. No abnormalities were seen on subsequent high-resolution CT scanning. Three days later, he experienced mild dyspnoea [time to reaction onset not clearly stated]. On further scanning, patchy ground-glass opacities were present with interlobular septum thickening. In the absence of microbiological abnormalities to indicate the presence of infectious disease, drug-induced pneumonitis was diagnosed. He was commenced on high-dose steroid therapy, and after 3 weeks his symptoms and CT scan abnormalities had almost disappeared. Author comment: "The pathogenesis of drug-induced pneumonitis remains unclear and mechanisms differ among drugs; however, neutrophils are considered to play an important role." Yamane T, et al.
