Acute cardiac failure: case reportIn a retrospective study, an elderly patient [age and sex not

stated] was identified, who developed acute cardiac failureassociated with rituximab, cyclophosphamide, doxorubicin, vincristine and prednisolone(R-CHOP) chemotherapy.

The patient, having a history of chronic heart failure, with anejection fraction of 40%, started receiving R-CHOP infusiontherapy for diffuse large B-cell lymphoma. The R-CHOPregimen included rituximab 375 mg/m2 on day 0,cyclophosphamide 375 mg/m2 on day 1 and day 2,doxorubicin 25 mg/m2 on day 1 and day 2, vincristine1.4 mg/m2 tapered to a maximal dose of 2mg on day 1 andprednisolone 100mg on day 1 to day 5 [routes not stated].Patients in the study were scheduled to receive R-CHOP cyclesrepeated every 3 weeks for a total of 6 cycles. The patientdeveloped acute cardiac failure [duration of treatment toreaction onset and outcome not stated].

Author comment: "While full-dose R-CHOP regimen isassociated with grade 3/4 cardiac toxicity of 7–9%"."[O]nlyone of our patients showed acute cardiac failure associatedwith chemotherapy."Kreher S, et al.
