Osteoporotic greenstick fracture: case reportA girl with osteosarcoma developed an osteoporotic
greenstick tibial fracture while receiving adjuvantchemotherapy with methotrexate, cisplatin, doxorubicinand ifosfamide [frequencies and routes not stated; not alldoses stated].
At 12 years of age, in May 2008, the girl was diagnosedwith high-grade osteosarcoma. In June 2008, shecommenced neoadjuvant chemotherapy with high-dosemethotrexate 12 g/m2, cisplatin and doxorubicin under theItalian Sarcoma Group OS1 protocol. In August 2008,3 weeks after her last cycle, she underwent tumourresection and reconstruction arthroplasty. Three weeksafter surgery, she started receiving adjuvant chemotherapywith methotrexate, cisplatin, doxorubicin and ifosfamide.Eight months after surgery, while still receivingchemotherapy, she presented with acute pain in the rightdistal tibia. X-rays revealed a distal greenstick tibial fracture.Contrast MRI demonstrated the fracture was not ametastatic lesion. Prior to the fracture, she had received acumulative methotrexate dose of 48 g/m2.
The girl underwent application of a plaster cast, whichwas worn for 40 days. She then wore a splint for a further4 weeks. Three months after the fracture occurred, x-rayrevealed healing. She remained disease-free and in goodhealth at 18 months after surgery.Zoccali C, et al.
