Reactions 1357 - 25 Jun 2011 S Antineoplastics Osteoporotic greenstick fracture: case report A girl with osteosarcoma developed an osteoporotic greenstick tibial fracture while receiving adjuvant chemotherapy with methotrexate, cisplatin, doxorubicin and ifosfamide [frequencies and routes not stated; not all doses stated]. At 12 years of age, in May 2008, the girl was diagnosed with high-grade osteosarcoma. In June 2008, she commenced neoadjuvant chemotherapy with high-dose methotrexate 12 g/m 2 , cisplatin and doxorubicin under the Italian Sarcoma Group OS1 protocol. In August 2008, 3 weeks after her last cycle, she underwent tumour resection and reconstruction arthroplasty. Three weeks after surgery, she started receiving adjuvant chemotherapy with methotrexate, cisplatin, doxorubicin and ifosfamide. Eight months after surgery, while still receiving chemotherapy, she presented with acute pain in the right distal tibia. X-rays revealed a distal greenstick tibial fracture. Contrast MRI demonstrated the fracture was not a metastatic lesion. Prior to the fracture, she had received a cumulative methotrexate dose of 48 g/m 2 . The girl underwent application of a plaster cast, which was worn for 40 days. She then wore a splint for a further 4 weeks. Three months after the fracture occurred, x-ray revealed healing. She remained disease-free and in good health at 18 months after surgery. Zoccali C, et al.
