Blastoschizomyces infection?: 6 case reportsSix patients with haematological malignancies developed
Blastoschizomyces [Trichosporon] infections afterchemotherapy.
The patients were aged between 26 and 67 years; fivewere male. Patient 4 had non-Hodgkin’s lymphoma withautogenic transplant of haematopoietic progenitors, whilethe rest had acute myeloid leukaemia. Patient 4 receivedcarmustine, etoposide, cytarabine and melphalan; theother five were treated with idarubicin, cytarabine andetoposide [dosage details not stated; duration of treatmentto reaction onset not clearly stated]. The patients hadreceived chemotherapy from 6 to 33 days previously.Duration of neutropenia ranged from 11 to 21 days. Themost common clinical manifestations were fever, skinlesions, jaundice and shock. Despite antifungalprophylaxis, Blastoschizomyces spp. was isolated fromblood and bronchoalveolar washings.
Treatment consisted of amphotericin B for 4 to 18 daysor voriconazole (Patient 3). Five patients died: four frominvasive fungal infection and one from Klebsiella sepsis.Patient 2, who received the longest course ofamphotericin B therapy, was cured.Batlle M, et al.
