Reactions 908 - 29 Jun 2002 S Antineoplastics First report of fatal gallbladder carcinoma in an elderly patient: case report A 67-year-old man developed carcinoma of the gallbladder after repetitive hepatic arterial chemoembolisation for the treatment of hepatocellular carcinoma. The man had received 9 courses of hepatic arterial chemoembolisation [drug and dosage not stated] over a 3-year period when he presented with a 15-day history of progressive jaundice and fever. He had a history of alcohol-related cirrhosis and hepatocellular carcinoma in the right lobe of his liver with atrophy of his left lobe. A cholangiogram revealed lesions of secondary sclerosing cholangitis. Placement of an endoscopic stent was carried out and was partially effective. He then underwent a surgical procedure which involved hepaticojejunal anastomosis with cholescystectomy. Pathological examination found transmural fibrosis, thickening and replacement of the man’s gallbladder wall with granulation tissue. There was adenocarcinoma infiltration of the serosa in his gallbladder. He died a few days later from septic shock. Author comment: ‘Our case report provides evidence for a direct link between chronic acalculous cholecystitis and cancer . . . Cholecystectomy has to be discussed in patients treated with repetitive courses of HACE [hepatic arterial chemoembolisation].’ Balian A, et al.
