Carbamazepine

Lupus erythematosus: case reportA 35-year-old man, who had received carbamazepine

[dosage not stated] for 20 years for epilepsy, complained ofchest pain, dyspnoea and reduced physical activity for3 months [duration of treatment before reaction onset notclearly stated]. After he was subsequently diagnosed with alarge pericardial effusion and had undergonepericardiocentesis (1200mL), viral perimyocarditis wassuspected. He received anti-inflammatory treatment and hissymptoms initially resolved. After 4 weeks, his pericardialeffusion recurred and livedo reticularis was evident.Serological examination showed antinuclear antibodies andantibodies against histones. He was diagnosed withcarbamazepine-induced lupus erythematosus. Aftercarbamazepine discontinuation and immunosuppressivetherapy initiation, he recovered completely.Wittchen F, et al.
