Reactions 1197 - 12 Apr 2008 S Oxcarbazepine Haemolytic anaemia in an elderly patient: case report A 75-year-old man developed haemolytic anaemia during treatment with oxcarbazepine for seizure disorders. The man, who was receiving oxcarbazepine [dosage not stated], was hospitalised with a few-day history of lethargy and dark urine discolouration [duration of treatment to reaction onset not stated]. Generalised pallor was evident on examination, and laboratory investigations revealed the following levels and values: WBC count 32/mm 3 , haemoglobin 4.6 g/dL, haematocrit 14%, reticulocyte count 9%, mean corpuscular volume 89fl, mean corpuscular haemoglobin 29 pg/cell, RBC distribution width 25, platelet count 305/mL, lactate dehydrogenase (LDH) 1314 U/L, total bilirubin 3.4 mg/dL and direct bilirubin 0.5 mg/dL. A peripheral blood smear showed polychromasia, microspherocytes and a few nucleated RBCs; 12 RBCs per high power field, haemosiderinuria and moderate bilirubin were revealed by urinalysis. The man received RBC transfusions. Drug-induced haemolytic anaemia was suspected after exclusion of other causes, and oxcarbazepine was stopped. His symptoms improved, his WBC count, haemoglobin level and haematocrit normalised, and reticulocyte count, bilirubin and LDH levels decreased.
