Haemolytic anaemia in an elderly patient: casereport
A 75-year-old man developed haemolytic anaemia duringtreatment with oxcarbazepine for seizure disorders.
The man, who was receiving oxcarbazepine [dosage notstated], was hospitalised with a few-day history of lethargy anddark urine discolouration [duration of treatment to reactiononset not stated]. Generalised pallor was evident onexamination, and laboratory investigations revealed thefollowing levels and values: WBC count 32/mm3, haemoglobin4.6 g/dL, haematocrit 14%, reticulocyte count 9%, meancorpuscular volume 89fl, mean corpuscular haemoglobin29 pg/cell, RBC distribution width 25, platelet count 305/mL,lactate dehydrogenase (LDH) 1314 U/L, total bilirubin3.4 mg/dL and direct bilirubin 0.5 mg/dL. A peripheral bloodsmear showed polychromasia, microspherocytes and a fewnucleated RBCs; 12 RBCs per high power field,haemosiderinuria and moderate bilirubin were revealed byurinalysis.
The man received RBC transfusions. Drug-inducedhaemolytic anaemia was suspected after exclusion of othercauses, and oxcarbazepine was stopped. His symptomsimproved, his WBC count, haemoglobin level and haematocritnormalised, and reticulocyte count, bilirubin and LDH levelsdecreased.
