Reactions 1283 - 9 Jan 2010 S Oxcarbazepine Exacerbation of seizures in children: 3 case reports Three girls (aged 2–8 years) with partial-onset epilepsy experienced an exacerbation of their seizures after starting oxcarbazepine [dosages and durations of therapy before reaction not stated]. Patient 1 developed staring spells with up-rolling of eyes and eyelid fluttering and became unresponsive after starting oxcarbazepine. She also experienced loss of tone resulting in a fall during some episodes. Generalised spike and wave activity was evident on an ictal EEG. Oxcarbazepine was discontinued and her epilepsy was managed with ethosuximide. After initiation of oxcarbazepine, patient 2 experienced an increase in seizure frequency and developed significant behavioural problems. An interictal EEG showed frequent independent and bilateral centro-temporal spike-wave activity during sleep. Oxcarbazepine was discontinued and she became seizure free under levetiracetam therapy. Patient 3 experienced a marked increase in seizure frequency (increasing to several per day) after starting oxcarbazepine. Generalised spike-wave activity was evident on an ictal EEG. Oxcarbazepine was discontinued and she became seizure free while receiving valproic acid. Harini C, et al.
