Reactions 1384 - 14 Jan 2012 S Benazepril Life-threatening angioedema in an elderly patient: case report A 65-year-old man developed life-threatening angioedema following treatment with benazepril. The man had a history of intermittent oedema of the face, eyes and lips, that was alleviated by hydroxyzine and prednisone. He was receiving benazepril for hypertension [dosage and route not stated]. Two weeks following prednisone withdrawal, he developed angioedema, dyspnoea and an erythematous rash [duration of treatment to reaction onset not stated]. Benazepril was discontinued, and the man was intubated. He received diphenhydramine, epinephrine [adrenaline] and methylprednisolone, and his rash and angioedema resolved. On follow up, he presented with facial angioedema and urticarial lesions. He received famotidine, prednisone, cetirizine and diphenhydramine [outcome not clearly stated]. Tests revealed an IgE receptor antibody titre of 12.4%, and a serum IgE level of 199 IU/mL. Author comment: "[O]ur patient had underlying chronic autoimmune urticaria with angioedema, which in the setting of [ACE inhibitor] use precipitated a life-threatening reaction." Kleva A, et al. Life-threatening angioedema in an elderly patient:case report
