Reactions 1333 - 8 Jan 2011 S Antineoplastics Acute renal failure associated with BKV reactivation: case report A man developed acute renal failure due to polyoma BK virus reactivation following multiple chemotherapy treatments for refractory T-cell lymphoma [time to reaction onset not clearly stated]. The man, who was diagnosed with peripheral T-cell lymphoma at the age of 55 years, received three courses of cyclophosphamide, vincristine, doxorubicin and dexamethasone (hyper-CVAD) with high-dose methotrexate and cytarabine. Due to a lack of durable response, he underwent salvage chemotherapy, receiving a cycle each of etoposide, vincristine, doxorubicin, prednisolone and cyclophosphamide (EPOCH), dexamethasone, etoposide, ifosfamide and carboplatin (DeVIC) and gemcitabine, dexamethasone and cisplatin (GDP) [dosages and routes not stated]; however, his disease continued to progress. Eight months after diagnosis, he developed fever and dyspnoea. Investigations revealed systemic complications of lymphoma and he received palliative chemotherapy with irinotecan 30 mg/m 2 /day on days 1-3 and dexamethasone 10 mg/day on days 4-6 [routes not stated], which relieved his dyspnoea. Ten days after starting this regimen, he developed urinary frequency and fever. He became anuric 5 days later, and his serum creatinine rose to 3.3 mg/dL. Tests led to a diagnosis of nephropathy associated with the reactivation of a polyoma (BK) virus infection. The man was started on haemodialysis 17 days after initiating chemotherapy, with a peak creatinine level of 7.2 mg/dL. He died of disease progression 3 months following diagnosis of acute renal failure. His family did not consent to an autopsy. Aoki K, et al.
