Reactions 1410 - 14 Jul 2012 S Antineoplastics Various toxicities in paediatric patients: 2 case reports Two patients with stage IV neuroblastoma developed various toxicities, including mucositis, gastrointestinal bleeding, hypertension and intracranial bleeding, after receiving iobenguane, carboplatin, melphalan and etoposide [routes not stated]. A 9-month-old girl received six courses of cyclophosphamide, vincristine, etoposide, cisplatin, dacarbazine, ifosfamide and doxorubicin and underwent incomplete resection of an adrenal mass. Prior to bone marrow transplantation, she received a conditioning regimen that consisted of iobenguane 100mCi (10 mCi/kg) on day -21, melphalan 45 mg/m 2 /day on days -8 to -5, etoposide 200 mg/m 2 /day [days of administration not clearly stated] and carboplatin 300 mg/m 2 /day on days -5 to -2. Grade 4 mucositis and massive GI bleeding were observed on day +15 and she died on day +18. An 8-year-old girl received six courses of the same regimen as the previous patient, as well as postoperative radiotherapy for a residual adrenal mass. As conditioning therapy for bone marrow transplantation, she received the same regimen, including iobenguane 100mCi (3.1 mCi/kg) on day -21. Grade 1 graft-versus-host disease was diagnosed on day +7. She experienced acute hypertension and intracranial bleeding on day +25 and died on day +31. Author comment: "Although mucositis and myeloablation was not observed with [iobenguane] as a single agent, combination therapies with drugs at myeloablative doses may increase toxicity.
