Reactions 895 - 30 Mar 2002 S Antineoplastics Femoral head osteonecrosis: 2 case reports Two men, aged 20 and 37 years, developed nontraumatic osteonecrosis of the femoral head following antineoplastic therapy for testicular cancer. The 20-year-old man received four cycles of cisplatin, vinblastine and bleomycin [dosages not stated]; he had received a total equivalent prednisone dose of 2820mg. Approximately 2 years after antineoplastic therapy, he reported right hip pain. Two years later, he reported a 3-month history of worsening hip pain, difficulty walking and back pain. An x-ray confirmed bilateral nontraumatic osteonecrosis. He underwent total hip replacement, and was well at follow-up 8 years later. The 37-year-old man received 2 cycles of etoposide, cisplatin and bleomycin and 2 cycles of cisplatin and etoposide [dosages not stated]. He had received a total equivalent prednisone dose of 2240mg. Nine days after finishing his antineoplastic therapy, he developed transient severe right hip pain and his gait was affected. 3.5 years later, he experienced persistent, progressive and debilitating pain in his right hip, and he was admitted to hospital. Investigations revealed nontraumatic osteonecrosis of both femoral heads. He underwent total hip replacement and was well at follow-up > 5.5 years later. Winquist EW, et al.
