Reactions 1298 - 24 Apr 2010 S Antineoplastics Stomatitis: case report A 53-year-old woman with multiple myeloma developed severe stomatitis following chemotherapy with vincristine, doxorubicin, dexamethasone and melphalan [time to reaction onset not stated]. The woman received cycles of vincristine, doxorubicin, dexamethasone chemotherapy [dosages and duration of therapy not stated], followed by bone marrow conditioning with melphalan 200 mg/m 2 [frequency and duration of treatment not stated] prior to autologous bone marrow transplantation (BMT). Five days prior to BMT (D-5), dental surgeons performed an examination of her oral mucosa, which was intact. From D-5 to D+2, she underwent preventive mucosal conditioning using laser treatment, but still developed grade 2 stomatitis on D+3. The woman received supportive therapy, which included topical corticsteroids and an antifungal mouthwash thrice daily, oral antiseptics, a lip moisturiser, artificial saliva and topical lidocaine before meals. Daily laser treatments continued, from which she derived significant pain relief. On D+5, her stomatitis worsened to grade 3. An ulcerated lesion on her upper lip was diagnosed as recurrent herpes labialis and another lesion at the back of her tongue was seen as pseudo-membranous candidiasis. By D+8, her stomatitis had progressed to grade 4, and she became unable to take food orally. She also developed renal failure which required two haemodialysis sessions. From D+35, her stomatitis gradually improved. A PCR assay on material taken from the one remaining lesion at the back of her tongue was negative for cytomegalovirus and herpes simplex virus. Forty-five days after BMT, her symptoms and oral lesions had completely resolved with no residual scarring. Pavesi VCS, et al.
