Reactions 1438 - 9 Feb 2013 S Antineoplastics Neutropenia, septic shock, and interstitial pneumonia in an elderly patient: case report A 65-year-old woman with squamous cell carcinoma of the breast developed neutropenia and septic shock during treatment with fluorouracil, epirubicin, and cyclophosphamide, and interstitial pneumonia during treatment with paclitaxel. The woman underwent a mastectomy and axillary lymph node dissection after diagnosis of breast cancer. She also had a history of dermatomyositis and was taking steroids [drugs not specified]. She received fluorouracil, epirubicin, and cyclophosphamide [routes and dosages not stated] as post- operative adjuvant chemotherapy. However, she discontinued this treatment after one course due to neutropenia, as well as septic shock and disseminated intravascular coagulation due to an infection of the inserted CV board. After recovery, her chemotherapy was changed to weekly paclitaxel [route and dose not stated]. However, she developed interstitial pneumonia after six courses of paclitaxel therapy and this was also discontinued. The woman made progress but after 1 month developed squamous cell carcinoma recurrence and went on to receive gimeracil/oteracil/tegafur [S-1]. She developed no metastasis or enlargement of the tumour and resection was performed. She also received radiotherapy. At last follow-up, she had no further recurrence and continued to visit hospital as an outpatient. Author comment: [T]he [fluorouracil, epirubicin, cyclophosphamide] therapy first selected was discontinued after one round, and then changed to weekly [paclitaxel] therapy.
