Timeline of FDA regulatory approval for metastatic melanoma. Between 1976 and 2011, dacarbazine chemotherapy (1976) and high-dose IL-2 (1998) were the only approved agents for the treatment of advanced melanoma. The number of approved agents more than tripled in the last three years with the approvals of ipilimumab (anti-CTLA-4 antibody) and vemurafenib (BRAF inhibitor) in 2011, dabrafenib (BRAF inhibitor) and trametinib (MEK inhibitor) in 2013, and pembrolizumab (anti-PD-1 antibody) in 2014. • interferon a-2b (IFN a-2b) w dużych dawkach został zarejestrowany w Stanach Zjednoczonych i Unii Europejskiej dla czerniaków w stopniu zaawansowania IIB–III, natomiast lek w małych dawkach zarejestrowano dla chorych w II stopniu w Europie

CTLA-4 and PD-1 pathway blockade. CTLA-4 blockade allows for activation and proliferation of more T-cell clones, and reduces Treg-mediated immunosuppression. PD-1 pathway blockade restores the activity of antitumor T cells that have become quiescent. A dual pathway blockade could have a synergistic effect, resulting in a larger and longer lasting antitumor immune response. CTLA-4-mediated inhibition of T cells. T cells are activated when TCRs bind antigen displayed in the MHC on antigen-presenting cells in concert with CD28:B7-mediated costimulation. CTLA-4-mediated inhibition of Tregs. One hypothesis of how CTLA-4 expression on Tregs can inhibit T-cell activation is depicted. PD-1-mediated inhibition of T cells. T cells recognizing tumor antigens can be activated to proliferate, secrete inflammatory cytokines, and resist cell death. Inhibitory BRAF/BRAF+MEK • Wemurafenib - u chorych z mutacją BRAF V600 - odpowiedź na
leczenie u 48% chorych vs. 5% otrzymujących dakarbazynę. Przeżycia wolne od progresji - różnica około 5 miesięcy. Przeżycia całkowite - różnica około 3 miesiące. Pomimo, że u większości chorych dochodzi do pojawiania się oporności na leczenie (mediana przeżyć wolnych od progresji wynosi 6-7 miesięcy), wyniki badań wykazały medianę przeżyć całkowitych u chorych na przerzutowe czerniaki na poziomie 13-16 miesięcy, co znaczenie przewyższa obserwowane wcześniej przeżycia w tej grupie chorych.
• Dabrafenib - mediana PFS wyniosła 6,7 miesiąca vs. 2,9 miesiąca dla dakarbazyny. Raportowana w 2013 roku mediana przeżyć całkowitych przy leczeniu dabrafenibem wyniosła 18,2 miesiąca.
• Najnowsze wyniki badań (COMBI-d, COMBI-v i coBRIM), wykazały, że u chorych na przerzutowe czerniaki z obecnością mutacji BRAF zastosowanie kombinacji inhibitora BRAF i MEK (dabrafenib z trametynibem lub wemurafenib z kobimetynibem) przynosi lepsze efekty niż monoterapia. Mediana przeżyć wydłużyła się do ok. 23-25 miesięcy, przy medianie PFS około 12 miesięcy.
anty-CTLA-4/antyPD-1/PD-L1 • Ipilimumab (anty-CTLA-4 ) - odsetek obiektywnych odpowiedzi na leczenie
ipilimumabem jest niewielki (około 10% chorych), długotrwałe korzyści odnosi ograniczona liczba chorych (20-25%), jednak charakteryzują się oni wieloletnimi przeżyciami (najdłuższe obserwacje sięgają 10 lat).
• Pembrolizumab lub niwolumab (anty- PD-1/PD-L1) - w warunkach klinicznych w monoterapii lub w kombinacji z ipilimumabem długotrwałą korzyść kliniczną u części chorych na zaawansowane czerniaki i znaczne odsetki odpowiedzi (sięgające 50%), przy przeżyciach rocznych na poziomie 70-80%. Stosowanie pembrolizumabu lub niwolumabu jest związane z odsetkiem przeżyć 2-letnich około 50-60% (mediana przeżyć przekracza 2 lata, odsetek przeżyć 3-letnich wynosi około 45%),.
• Badanie kliniczne porównujące skuteczność niwolumabu w monoterapii, ipilimumabu w monoterapii oraz połączenia obu leków - niwolumab okazał się bardziej skuteczny niż ipilimumab (mediana przeżycia wolnego od progresji choroby wynosiła odpowiednio 6,9 wobec 2,9 miesięcy). Najskuteczniejsza okazałą się kombinacja leków; mediana przeżycia wolnego od progresji choroby to 11,5 miesięcy. Kombinacja leków była najlepszą opcją w przypadku obecności ekspresji PD-L1 w tkance nowotworowej poniżej 5%. W przypadku ekspresji PD-L1 powyżej 5% wyniki leczenia niwolumabem w monoterapii lub w skojarzeniu z ipilimumabem były porównywalne. Nie przedstawiono jednak czasu przeżycia całkowitego
