Instructions for completing this Assessment To be eligible for Verification of Competency (VOC) you must hold a Licence,

Qualification Card, Competency Certificate, or Ticket for the specific High Risk

Work or Plant operator tasks. This must have been obtained in a manner

acceptable to the OHS Regulator in the state or territory in which it was issued.

A copy of this, along with other supporting documents must be provided to Your Licence P/L (see "Supporting Documents needed" at the bottom of this page for more information).

Each section of this document is for an element of the national industry

competency standard related to the efficient and safe conduct of your work.

Topics covered in this self-assessment include planning and preparing for High Risk Work, pre-operational checks, carrying out work operations competently and safely, relocating plant and equipment, post-operational checks, maintenance and cleaning up.

Please go through each section of the document very carefully, and complete

the details fully. This record is a legal document and must be signed, dated,

and returned to the RTO as per the rules applicable to the VET training system.

Upon�completion of tKLV�process (including theory assessment, practical self-assessment and reference/declaration document),you will be able to download an Interim Certificate of Completion

from your emails. Dependent upon the procedures on your worksite, this certificate can be used to replace or support any

required competency test.

Your Statement of Attainment and VOC card will then be sent to you by mail from our affiliate Registered Training Organisation.
