Reactions 1348 - 23 Apr 2011 S Oxcarbazepine Seizure worsening: case report A 19-year-old woman with Jeavons syndrome developed oxcarbazepine-induced worsening of seizures. The woman, who had a history of seizures presented with a 1-year history of worsened seizures. She had a generalised tonic-clonic seizure every month. A few seconds before the events, she developed eyelid flutter followed by myoclonic jerks involving her upper limbs. The worsening of her seizures coincided with the initiation of oxcarbazepine 20 mg/kg [frequency, indication and route not stated]. Evaluation revealed inter-ictal generalised fast polyspikes during wakefulness and sleep. She had one episode of prolonged eyelid myoclonic status, which progressed to multifocal myoclonic jerks and generalised tonic-clonic seizure. EEG in bright light, showed discharges occurring consistently after closing the eyes, but not in darkness. A diagnosis of Jeavons syndrome worsened by oxcarbazepine was made. Oxcarbazepine was tapered and the woman started receiving levetiracetam. EEG showed reduced interictal discharges. At 4-month follow-up, she reported one episode of brief absence with eyelid myoclonia on awakening. Menon R, et al.
