First report of hypersensitivity with identification ofhuman liver autoantibodies: case report
Researchers form the University of Liverpool, UK, suggest’that serum from all patients with anticonvulsant-inducedhepatic injury should be screened for the presence ofautoantibodies in order to help elucidate the mechanism ofthis rare, but potentially life-threatening adverse reaction’.
Fever developed in a 16-year-old man 4 weeks aftercarbamazepine 400 mg/day was initiated to control epilepsy. Arash occurred 1 week later, and jaundice appeared withinanother week
Liver enzyme function was elevated and eosinophilia wasobserved Carbamazepine-induced hepatotoxicity wassuspected and carbamazepine was withdrawn. The patientrecovered within 6 weeks.
Serum samples taken at the time of this patient’s reactionrevealed autoantibodies recognising a protein band fromhuman liver microsomes. Further samples taken 2 and 6months after the reaction showed a diminished titre for theseantibodiesPirmohamed M, et al.
