Reactions 390 - 29 Feb 1992 Carbamazepine First report of hypersensitivity with identification of human liver autoantibodies: case report Researchers form the University of Liverpool, UK, suggest ’that serum from all patients with anticonvulsant-induced hepatic injury should be screened for the presence of autoantibodies in order to help elucidate the mechanism of this rare, but potentially life-threatening adverse reaction’. Fever developed in a 16-year-old man 4 weeks after carbamazepine 400 mg/day was initiated to control epilepsy. A rash occurred 1 week later, and jaundice appeared within another week Liver enzyme function was elevated and eosinophilia was observed Carbamazepine-induced hepatotoxicity was suspected and carbamazepine was withdrawn. The patient recovered within 6 weeks. Serum samples taken at the time of this patient’s reaction revealed autoantibodies recognising a protein band from human liver microsomes. Further samples taken 2 and 6 months after the reaction showed a diminished titre for these antibodies Pirmohamed M, et al.
