Reactions 1430 - 1 Dec 2012 S Antineoplastics Pneumatosis intestinalis: case report An 18-year-old man developed pneumatosis intestinalis following treatment with prednisolone, daunorubicin, vincristine, asparaginase and methotrexate [dosages and not all routes stated] for B lymphoblastic leukaemia. The man was hospitalised for consolidation chemotherapy; two months earlier, he received prednisolone, vincristine, asparaginase, intrathecal methotrexate and daunorubicin, an induction regimen, for B lymphoblastic leukaemia. Routine x-rays showed intraperitoneal free air with gas collection along the colon wall. Examination found mild abdominal distension. Tests showed the following values: haemoglobin 9.9 g/dL, leukocyte count 8.0 × 10 9 /L (87% neutrophils) and platelet count 278 × 10 9 /L. Extramural and intramural gas of the transverse and ascending colon was observed by CT in addition to pneumoretroperitoneum and pneumoperitoneum. He was diagnosed with pneumatosis intestinalis. Ceftazidime, teicoplanin and metronidazole were initiated. The man’s condition subsequently improved; repeat imaging, 2 weeks later, revealed significant regression of pneumatosis intestinalis with resolution noted after 4 weeks. Symptoms of acute lymphoblastic leukaemia ensued after a few days necessitating successful readministration of vincristine.
