Reactions 1405 - 9 Jun 2012 S Antineoplastics Oesophageal stricture (first report for ifosfamide): case report A 14-year-old boy developed oesophageal stricture following antineoplastic therapy [frequencies and routes not stated] for osteosarcoma of the left distal femur. The boy received four courses of chemotherapy with cisplatin 167mg and doxorubicin 42mg. Despite prophylactic therapy, he reported severe nausea and vomiting, as well as hearing impairment due to cisplatin. After the initial four courses, treatment was changed to a regimen containing etoposide 84mg, ifosfamide 4200mg and methotrexate 28mg, of which he received two courses. Five months after diagnosis, he underwent tumour resection and total knee replacement. He then received six courses of the second chemotherapy regimen over a 5-month period. He experienced nausea and vomiting during this time. Four weeks after the 6-course chemotherapy, he reported dysphagia and was unable to eat solid foods [durations of treatment to reaction onset not clearly stated]. Investigation showed stricture of the distal oesophagus, with a slightly reddish and inflamed mucosa. Chemotherapy-related oesophageal stricture was diagnosed. Balloon dilatation was performed over 2 months and the oesophagus diameter increased by 12mm. The boy’s dysphagia improved and he was able to ingest solid food. At last observation, the oesophagus was normal with no strictures. Author comment: "[I]n our case, the oesophageal stricture seems to be a result of chemotherapy." Ishimaru D, et al.
