Drug hypersensitivity syndrome in an elderlypatient: case report

An 82-year-old woman developed drug hypersensitivitysyndrome following treatment with carbamazepine fortrigeminal neuralgia.

The woman was hospitalised with pyrexia, exanthemaand lymphadenopathy; three weeks prior, carbamazepinewas initiated [route, dosage and time to reaction onset notstated]. Examination found disseminated maculopapulareruption in addition to occipital and axillarylymphadenopathy. Tests revealed leucocytosis,eosinophilia, lymphocytosis and thrombocytosis inaddition to raised levels of lactate dehydrogenase and β-2microglobulin; a low mean corpuscular volume was alsodetected. Splenomegaly and lymphadenopathy wereobserved on echosonography. She was diagnosed withcarbamazepine-induced hypersensitivity syndrome. B celllymphoma was also identified based on bone marrowbiopsy and blood flow cytometry results.

Carbamazepine was withdrawn, and corticosteroidswere initiated. The woman’s condition subsequentlyimproved.Plavsic A, et al.
