☒ I have been informed that, according to Art.24 paragraph 1 of The Act of 29 August 1997 on the Protection of Personal Data (uniform text: Journal of Laws of the Republic of Poland 2002 No. 101, item 926 with further amendments),
1. the administrator of the data provided when placing the order is Czerny Marian Firma Prywatna GREG located in Gliwice (44-100), ul. Wrocławska 10, hereinafter referred to as GREG, 2. personal information will be processed for contractual purposes as well as marketing purposes of the products and services offered by GREG, 3. customer has the right to gain access to the their own personal information, modify it and to request GREG to stop using it as well as to object to the information being used, 4. providing personal information is voluntary; however, refusing to provide it or the lack of consent for its processing make it impossible for the order to be placed.

□ 1. Collecting the order in person.
□ 2. Shipping the order on Customer's request. The parties of the agreement confirm that the place of the realization of the order by the Contractor is theContractor's headquarters located in Gliwice (ul. Poezji 19) and the order is considered to be completed by theContractor at the moment of the pick-up of the goods by the carrier who is obliged to deliver the goods to the address specified by the Customer in the order placed with the Contractor. The order is considered to be completed by the Contractor at the moment of the pick-up of the goods by the carrier and the Contractor is not liable for any damage to the goods or their loss since their release; the liability is transferred to the carrier. Due to Contractor's long-term cooperation with carriers, the Contractor organizes shipment of the goods on behalf ofthe Customer. A waybill with the Customer put in as the sender is to be considered a proof of ordering shipmentof the goods (as well as realizing the order by the Contractor). Shipping costs are included in the price of the order agreed with the Customer and are paid to the carrier by the Contractor.
□ 3. The Customer chooses the carrier for the shipment of the goods.
□ 4. On Customer's request, the Contractor organizes the shipment of the goods to the address specified by the Customer.
□ 5. The Contractor ships goods by their own means of transport.
□ I hereby agree to the samples of the ordered goods being used for marketing purposes by Czerny Marian Firma Prywatna GREG at any time and in any form.
