INTRODUCTIONSWEPCO owns and operates a Class 3N Solid Waste Landfill (Landfill) located in Fulton County,Arkansas. The Landfill address is 3711 Highway 355 S, Fulton, Arkansas 71838. The subjectLeachate Collection Pond (LCP) supports the landfill with leachate storage. Construction of the LCPwas completed in July 2012 as part of the waste disposal facility permitted by the ArkansasDepartment of Environmental Quality (ADEQ) under Solid Waste Permit No. 0311-S3N. FIGURE 1in APPENDIX A illustrates the layout of the Turk facility. FIGURE 2 illustrates the location andorientation of the LCP in relation to the facility’s waste management area. This construction qualityassurance (CQA) report summarizes testing and documentation activities performed by TerraconConsultants, Inc (Terracon) during the improvements of the LCP.

BACKGROUNDThis LCP consists of approximately 7.8 acres of composite bottom liner system and protective coverthat was completed in July 2012. The LCP has an underdrain system, 2-foot clay liner, 60-miltextured HDPE geomembrane, covered with a geotextile, and a protective cover layer. Thecomposite liner system slopes were constructed on a 3 H: 1 V while the protective cover layer wasfinished on a 4 H: to 1 Vertical. The protective cover system on the west slope of the LCP hadslipped down the embankment. On June 29, 2018 the ADEQ approved the installation of a 60-milrub sheet on the west slope and additional gravel drains on the south and southeast slopes to repairthe current system (See APPENDIX F).

PROJECT DESCRIPTIONTerracon Consultants, Inc. (Terracon) observed and documented the repairs to the geomembraneliner system and installation of a rub sheet along the west slope of the leachate pond located northof Cell 2. Also, the protective cover on the west slope was removed and placed on the south slope.Additional gravel drains were also installed on the south and southeast slopes.

3.1 KEY PERSONNEL

Boral CM Services, Inc. (Boral) was contracted to perform the earthwork for the project that includedexcavation of protective cover from the west slope, transfer of protective cover to the south slope,and installation of gravel letdown structures. Repair to the HDPE liner and installation of the rubsheet was contracted to Environmental Specialties International (ESI).

LEACHATE COLLECTION POND IMPROVEMENTSBoral began removing the protective cover from the west slope. The material on the west slope wastransported to the south slope (See Photo 1 in APPENDIX E). Boral cleaned the exposedgeomembrane on the west side. Terracon walked the west side and documented any damage notedon the geomembrane (See Photo 2). ESI arrived on site and cleaned and prepared the existingliner material prior to beginning repairs by using brooms, shovels, and grinders. The geosyntheticrepairs and rub sheet installation occurred during the period of August 27-28, 2018 (See Photos 3,4, 5, 6). The daily logs describing Terracon observations are in APPENDIX B. The liner repairswere made by ESI using 60-mil HDPE textured geomembrane patches where necessary (SeeFIGURE 2). The rub sheet was installed and its panel layout is presented on FIGURE 3. All repairswere made with an HDPE extrusion welder. The liner repairs were nondestructively tested using avacuum box. The documentation of the geomembrane liner repairs is shown in the summary tableslocated in APPENDIX C. The documentation of the rub sheet installation is located in APPENDIXD.

Boral also excavated gentle swales for stabilized letdown structures into the protective cover on thesouth slope. The south slope was flattened from the original 4 H: 1 V slope to an approximately 6H: 1 V slope (See FIGURE 4). Geotextile was installed above the let down swale areas (See Photo9). Protective letdown stone was then installed above the geotextile (See Photos 10 and 11).

Boral constructed a geosynthetics anchor trench at the top of slope for keying in of the rub sheetgeomembrane. Boral then removed the temporary sandbags and installed clay rich soil in the rubsheet anchor trench (See Photos 12 and 13). The south slope was revegetated (See Photo 14).
