1. This spreadsheet has 3 "or#sheets entitled $IN(T)*+TI,N($ $M%(*)%M%NT($ and $),*/(*MM)'.$ 'ou can mo0e to each of these using the ta&s at the &ottom of the screen. The$M%(*)%M%NT($ "or#sheet is "here childrens measurements can &e added and the BMIcalculations are done. The $),*/ (*MM)'$ "or#sheet summaries the pre0alence of o&esity ando0er"eight for all children "ho had 0alid measurements.

2. Information on &irth date date measured se height and "eight must &e entered into the &o on theM%(*)%M%NT( "or#sheet. (ee the eample at the &ottom of these instructions.

3. %ach child4s se should &e entered as 5M6 to indicate male or 576 to indicate female. This field is notcase sensiti0e.

. %ach child4s &irth date should &e entered using month9day9year format. 7or eample :uly 1; 1<<="ould &e entered as ;91;91<<=.

>. %ach child4s date of measurement should &e entered using month9day9year format. 7or eample,cto&er > 2!!; "ould &e entered as 1!9>92!!;. The child4s age must &e less than 2! years. The BMIcalculations contained in this spreadsheet are not meant to &e used "ith adults 2! years of age andolder.

=. ?eight can &e entered in feet and inches. 7eet can &e omitted if the full num&er of inches is included.7or eample feet 3 19 inches may &e entered as either a $$ in the $7eet$ column and a $3.2>$ in the$Inches$ column or may &e entered "ith the $7eet$ column &lan# and a $>1.2>$ in the $Inches$ column.

In %cel 2!!3 you can clic# the /rint icon from the tool&ar or select[File] and then the [Print] option. @hen you print the first ! ro"s on the spreadsheet "ill print &ydefault. If you need to print more ro"s than ! you should define a ne" print area. 'ou can do this &yselecting the cells that you "ant to print. fter you clic# to print select the radio &utton la&eled

[Selection] from the print dialogue andnot
